182. Comparing Outcomes Of Traditional Lip Repair Vs. Early Cleft Lip Repair On A National Scale Article Swipe

PURPOSE: Cleft lip (CL) is one of the most common congenital anomalies, and has traditionally been repaired surgically when the patient is between three and six months of age. However, recent single-institutional studies have demonstrated the efficacy and safety of early cleft lip repairs (ECLR) during the neonatal period. This study seeks to evaluate the outcomes of ECLR (repair < 1 month) vs. traditional lip repair (TLR) on a national scale. METHODS: The American College of Surgeons National Surgical Quality Improvement Program Pediatric Date File was used to query patients who underwent CL repairs between 2012-2022. Multiple logistic regression was performed to model the association of ECLR vs. TLR with death within 30 days, dehiscence rates, and rates while controlling for the following covariates: prematurity, American Society of Anesthesiologists (ASA) class (stratified by severity), age, and race. Secondary outcomes were operation time and bleeding complications. RESULTS: Upon retrospective review, 863,860 patients were identified in the database, 10,132 of whom were determined to have CL. When controlling for the aforementioned covariates, multiple logistic regression determined that neonatal status was not significantly associated with the likelihood of death within 30 days of repair (p=0.480). Similarly, neonatal status was not significantly associated with the likelihood of dehiscence following surgical correction (p=0.348). Operation time was significantly greater in the non-neonatal cohort (TLR: 122 minutes vs. ECLR: 93 minutes, p<0.001). There was no significant difference in the incidence of bleeding complications between the two cohorts (p=0.105). CONCLUSION: The findings of this study provide nationwide evidence that ECLR does not lead to an increased risk of adverse outcomes or longer surgeries compared with TLR. The results provide evidence that ECLR can be a viable and beneficial option for patients, as an earlier intervention would beget decreased impact of their anomaly on development, particularly with regard to speech and language. Additional studies are necessary to elucidate this critical relationship and other longer-term impacts of this earlier intervention.
