症例は69歳，女性．40代に，臍ヘルニアに対してメッシュ挿入によるヘルニア修復術を受けた．2014年頃より，単純性腸閉塞にて短期間に入退院を繰り返したため手術適応と判断した．CT所見からは腹壁への小腸の癒着が原因と考えられたが，元来の高度脊柱後弯やヘルニア手術歴のため，術前検査だけでは皮切や修復方法の詳細な計画が困難であった．そこで診断的腹腔鏡検査を施行したところ，腹壁は気腹下でも亀背およびメッシュの影響で背側へV字状に屈曲し腸間膜と近接していた．また，その間隙に挟まれるような形で小腸が腹壁と癒着し閉塞起点となっていたことが判明した．腸管の修復のみならず腹壁の形成も必要だと判断し，後日開腹による小腸部分切除+メッシュ除去+腹壁形成術を行った．要因が一元的ではない腸閉塞症例の治療にあたり，診断的腹腔鏡検査が方針決定の大きな助となり，術後の経過も良好であったため報告する．
