背景.近年,免疫チェックポイント阻害薬(immune checkpoint inhibitor:ICI)は非小細胞肺癌の術前補助化学療法(neoadjuvant chemotherapy:NAC)に使用されるようになった.肺癌診療医は過去に様々な免疫関連有害事象(immune-related adverse events:irAE)を経験してきたが,irAEとしての胸膜炎は稀であり,ICIを併用したNAC後に発症した報告はない.症例.63歳男性.右上葉肺扁平上皮癌(cT2aN1M0,cStage IIB)に対してICI併用NACを行った.右上葉切除を行い術後病理では病理学的完全奏効だったが,術後第22病日に発熱と呼吸困難を訴えた.右胸水貯留を認め,胸水検査では細菌感染の所見がなく,胸水中のリンパ球増多を認めたことからirAE胸膜炎と診断した.プレドニゾロンの投与により胸膜炎は軽快した.結論.ICI併用NACを行い,外科切除が行われた後の胸水貯留は,補腔性胸水や細菌性胸膜炎以外にもirAE胸膜炎を鑑別に挙げる必要がある.
