A case of radically resected pulmonary adenocarcinoma with pulmonary embolism during induction chemotherapy Article Swipe

肺塞栓症は重篤な周術期合併症の一つであり，担癌状態や化学療法はそのリスク因子と考えられている．今回導入化学療法中に肺塞栓症を発症するも，抗凝固療法後に根治的手術を行い得た一例を経験したので報告する．症例は73歳女性．既往は脂質代謝異常症．健診胸部レントゲンで異常陰影を指摘され，精査の結果，肺腺癌，cT2aN2M0，IIIA期の診断を得，導入化学療法後の手術を予定した．シスプラチン，ペメトレキセド，ベバシズマブによる治療を3コース行った後，造影CTで右肺動脈本幹に広範な血栓を認め肺塞栓症と診断した．抗凝固療法開始後，縮小した残存血栓は認めたが，器質化している可能性が高いと考えられ，開胸右下葉切除術およびリンパ節郭清（ND2a-2）を施行した．残存血栓の約79.5%は切除肺の肺動脈内に存在した．術後新規肺塞栓症は認めず，術後6日目に軽快退院となった．
