A debate. �Orbitando� en torno al abordaje de la angina estable. Perspectiva del cl�nico Article Swipe
PREGUNTA: ¿Cuáles son, en su opinión, las conclusiones que se pueden extraer de los dos estudios ORBITA1,2? RESPUESTA: Los estudios ORBITA se centran en un aspecto concreto del tratamiento de los pacientes con síndrome coronario agudo: el beneficio en el alivio sintomático de la angina1,2. El estudio ORBITA1 es un ensayo clínico doble ciego, aleatorizado y multicéntrico en el que se analizó, en 230 pacientes con enfermedad grave de un vaso y síntomas isquémicos, si la intervención coronaria percutánea (ICP) en comparación con un procedimiento placebo se asociaba a una mejoría en el tiempo de ejercicio libre de angina1. Tras 6 semanas de seguimiento no se detectaron diferencias estadísticamente significativas en el objetivo primario (diferencias en el incremento de ejercicio en la prueba de esfuerzo) entre ambos grupos. El estudio ORBITA2, que es un ensayo clínico doble ciego y multicéntrico, incluyó 301 pacientes con angina de esfuerzo que fueron aleatorizados a tratamiento con ICP o a placebo2. La metodología difiere de la del estudio ORBITA: todos los pacientes dejaron la medicación antianginosa 2 semanas antes de la aleatorización, y solo fueron incluidos si en ese periodo presentaban angina (evaluada por una compleja puntuación a través de una aplicación móvil)3.
