A Descriptive Analysis of Facial Nerve Grafting Outcomes in Head and Neck Cancer Patients Undergoing Free Flap Reconstruction. Article Swipe

Background: Reconstruction in head and neck cancer patients poses a unique challenge, in part due to their need for radiation therapy. In this retrospective cohort study, we attempt to characterize facial nerve grafting outcomes within this population. Methods: We conducted a retrospective review of all head and neck cancer patients at a single, large, multidisciplinary cancer center from 2000-2017 who underwent facial nerve grafting and simultaneous free flap reconstruction. Data points collected included demographics, comorbidities, radiation timing, type of oncologic resection, donor nerve, nerve graft length, type of free flap performed, and concurrent adjunct procedures performed. Descriptive analysis was conducted, and a 2-tailed t-test was utilized to generate p-values. Results: 46 total patients (51 distinct nerve grafts) were included. Average patient age was 59.5. Most patients underwent total parotidectomy (80.4%) and temporal bone resection (73.9%) and received radiation therapy after grafting (78.3%). Nerve grafts (median length of 4cm) were performed using 8 different donor nerves, with the most common being motor nerve to vastus lateralis (Table 1). Most patients (56.5%) had a concurrent static sling procedure and concurrent periocular procedures (80.4%). Assessing facial region and corresponding nerve branch grafted, average global HB scores were 2.50 (frontal), 2.41 (zygomatic), 2.46 (buccal), and 2.54 (mandibular), and final postoperative HB scores most commonly fell into the range of 1-2. Average global HB scores were significantly worse postoperatively but were not significantly affected by radiation (Table 2). The average improvement in HB scores for patients who did not receive post-operative radiation was +1.28, while the average change in HB scores for patients who received post-operative radiation was +0.34. The periocular region was the only facial region with a significant change in HB scores (HB 2.0 vs 0.27, p=0.04) (Table 3). Conclusion: Our preliminary retrospective data suggest that facial nerve grafting with concurrent soft tissue reconstruction can help restore some facial nerve function for patients.
