A trajetória da saúde pública no Brasil reflete transformações profundas ocorridas no campo político, social e institucional desde o período imperial até a consolidação da República. Este estudo tem como objetivo analisar a evolução das políticas e estruturas de saúde pública no país, desde o século XIX até os dias atuais, destacando os marcos históricos, os modelos assistenciais predominantes e os fatores que condicionaram sua reformulação ao longo do tempo. Trata-se de uma pesquisa bibliográfica de caráter qualitativo, com base em fontes científicas publicadas entre os anos de 2018 e 2025. A análise evidenciou que a saúde pública no Brasil foi historicamente marcada por rupturas e avanços, frequentemente condicionados por crises sanitárias, mudanças governamentais e pressões sociais. Conclui-se que a consolidação de um sistema público universal, como o SUS, foi fruto de processos históricos complexos e contínuos, exigindo ainda esforços permanentes de fortalecimento, financiamento e participação social.
