A G1 9-mm Rectal Neuroendocrine Neoplasm with Lymph Node Metastasis Article Swipe

10mm未満の直腸神経内分泌腫瘍（NEN）のリンパ節転移率は低率であるが，微小病変でもリンパ節転移を認める例も報告されており治療戦略は確立されていない．今回，リンパ節転移を伴った９mmの直腸神経内分泌腫瘍G1の１例を経験した．症例は70歳男性，大腸内視鏡検査にて直腸Rb部に小指頭大の粘膜下腫瘤を指摘された．腫瘤は頂部に陥凹を伴っており，生検にてNEN G1の診断となった．腹部CT検査にて10mm大のリンパ節腫大を指摘され腹腔鏡下低位前方切除術（D2郭清）を施行した．病理所見では最大径９mmで深達度はSMであった．核分裂像数は２/２mm2未満でKi-67 指数は３％未満とNEN G1であったがly1とリンパ管侵襲陽性であった．腸管傍リンパ節（251）にて３/７とリンパ節転移陽性を認めた．本症例のように微小病変であっても治療方針の決定には画像評価を行うことが必要と考えられた．
