Pabrik Tahu Pak Budi merupakan industri kecil menengah yang mengolah kacang kedelai menjadi tahu. Dalam proses produksi masih terdapat kesalahan-kesalahan yang dapat mengakibatkan adanya produk tahu yang rusak/cacat. Oleh karena itu, perlu dilakukan pengendalian terhadap kualitas tahu pada Pabrik Tahu Pak Budi. Tujuan penelitian ini untuk mengukur kinerja dan tingkat cacat produk tahu yang terjadi menggunakan metode Six Sigma, mengidentifikasi faktor penyebab produk cacat, memberikan usulan saran perbaikan dalam upaya meminimalisir produk cacat menggunakan metode Fuzzy FMEA. Six Sigma merupakan metode peningkatan kualitas yang banyak digunakan oleh perusahaan dan organisasi, dengan mengedepankan konsep bahwa cacat produk hanya 3,4 untuk setiap satu juta produk yang dihasilkan. Metode FMEA (Failure Mode and Effect Analysis) adalah metode analisis terjadinya resiko atau kegagalan pada proses. Keterbatasan pada metode FMEA dapat diselesaikan dengan penerapan logika fuzzy. Hasilnya berdasarkan perhitungan nilai DPMO dan level sigma cacat tekstur memperoleh level sigma 3,487, cacat warna memperoleh level sigma 3,767 dan cacat ukuran memperoleh level sigma 3,721. Faktor yang menyebabkan produk tahu cacat/rusak berdasarkan fishbone adalah metode, manusia, material dan mesin. Nilai FRPN tertinggi adalah 720 pada cacat tekstur dengan penyebab tidak adanya SOP. Sehingga usulan perbaikan yang dapat diberikan adalah pembuatan SOP.
