Introdução: O câncer de orofaringe constitui importante desafio de saúde pública e o cirurgião-dentista desempenha papel central na detecção precoce, dada sua posição estratégica na atenção primária. O objetivo geral deste estudo é analisar a distribuição temporal, geográfica e por sexo dos casos de câncer de orofaringe no Brasil entre 2015 e 2024, utilizando dados do DATASUS (Painel-Oncologia – Brasil). Metodologia: Foram analisados dados secundários do DATASUS (Painel-Oncologia – Brasil) referentes ao período 2015–2024, contemplando casos de neoplasia maligna da orofaringe (CID-10 C10) por sexo e Unidade da Federação. Aplicaram-se estatísticas descritivas, médias móveis, ANOVA one-way e two-way, teste qui-quadrado e correlações de Pearson. Resultados: Registraram-se 41.429 casos no período, com média anual de 4.142,9 (±762,7), mínimo de 2.803 em 2015 e máximo de 4.905 em 2021. Homens representaram 83,13% (34.440) e mulheres 16,87% (6.989). São Paulo concentrou 20,6% dos casos (854,7/ano), enquanto Amapá e Roraima juntos não atingiram 2% do total. A ANOVA revelou efeito geográfico predominante (η²≈0,95). O qui-quadrado mostrou associação significativa entre sexo e estado (χ²=326,84; p<0,001; V=0,089). As correlações entre casos masculinos e femininos foram elevadas (r=0,954–0,980). Conclusão: O estudo evidencia que o câncer de orofaringe no Brasil é condicionado principalmente pela geografia e pela predominância masculina, reforçando a necessidade de fortalecer políticas públicas de prevenção e consolidar o papel do cirurgião-dentista no diagnóstico precoce.
