Cognitive training associated with transcranial direct current stimulation in the treatment of age-related mild cognitive impairment: a systematic review of randomized clinical trials Article Swipe

Background: Cognitive training (CT) is an established therapy for cognitive decline in ,ild cognitive impairment (MCI), however, there are other non-pharmacological alternatives, such as transcranial direct current stimulation (tDCS). A novel approach suggests combining both CT and tDCS, in an attempt to augment its effects on patient cognitive performance. Objective: To synthesize evidence on the efficacy of combined cognitive training and transcranial direct current stimulation treatment of age-related mild cognitive impairment. Methods: A systematic review was conducted using the following databases: Medline, Embase, Cochrane, Web of Science, and BVS. The search terms utilized included "Transcranial Direct Current Stimulation," "Mild Cognitive Impairment," and "Adult," as well as their entry terms, combined with Boolean operators OR and AND. Only randomized controlled trials (RCTs) were included in the review. No language or date filters were applied during the search. The studies were independently analyzed by two reviewers according to eligibility criteria. The articles were screened using a computerized review software and the data obtained was organized in an online spreadsheet. The inclusion criteria were randomized controlled trials that compared the use of tDCS combined with CT versus sham-tDCS in adult patients with age-related MCI. The intervention group in the selected studies involved the use of tDCS with electrodes positioned in the left dorsolateral prefrontal cortex with a direct current of 2mA, on average, combined with computerized cognitive training. Results: A total of 381 articles were found, of which 12 studies were included, according to eligibility criteria, for analysis, totaling 440 patients. None of the studies included were conducted in South America. Overall, the tDCS + CT group obtained a greater improvement in the parameters analyzed (everyday memory, executive functions, working memory) compared to the sham tDCS group. However, most of the studies did not find statistically significant differences between the groups analyzed. Conclusion: Although tDCS combined with cognitive training was able to improve patient’s overall cognitive performance, these results were not statistically significant in comparison to controls. However, study methods diverged substantially, especially regarding the protocol for application of active and sham tDCS.
