D-31 | Impact of Frailty on Mitraclip Outcomes in the Patients With Severe Mitral Valve Regurgitation: A Nationwide Readmission Database Analysis Article Swipe

Frailty has been linked to worse outcomes, longer hospital admission, higher complications, and mortality. The impact of frailty on the transcatheter Mitral valve repair (MitraClip) is further explored in this study. We used the National readmission database from 2016-2018 using ICD-10 codes to compare different outcomes between the low-intermediate frailty (LIF) vs (versus) the high frailty (HF) groups who underwent Mitraclip. The total participants included were 16,591 (HF 504 & LIF 16,087). The average ages were 78±10 and 79±11 in the LIF vs the HF, respectively. The average length of stay (LOS) was higher in the HF 20±17 compared to 4.5±9 in the LIF. Also, the HF had higher statistically significant hospital mortality in comparison with the LIF (16.3% vs 1.8%), higher incidence of acute kidney injury (AKI) (70.4% vs 15.8), congestive heart failure (CHF) (94% vs 81.2%), stroke (6.3% vs 0.7%), myocardial infarction (MI) (8.2% vs 1.8%), mechanical circulatory support (MCS) (9.2% vs 1.9%), major adverse cardiac and cerebrovascular events (MACCE) (36.9% vs 6.1%) (p<0.001 for all). The data also shows higher readmission rates within 30 and 180 days in the HF compared to LIF (20.4% vs. 14.4% & 23% vs. 18.3%, respectively). However, readmission rates within 90 days in the two groups are similar (21%). MitraClip with high frailty is associated with worse outcomes, higher readmission, longer LOS, and longer recovery than the low-intermediate frailty group.
