DESAFIOS NO MANEJO DE ANEMIA FERROPRIVA GRAVE EM PACIENTES IDOSOS: RELATO DE CASO Article Swipe

Introdução/Objetivo: A anemia ferropriva causada pela deficiência de ferro, é a carência nutricional mais prevalente na saúde pública. Essa doença atinge todas as classes sociais, com maior evidência em crianças e mulheres, podendo atingir implicações graves. O objetivo deste trabalho é relatar o caso de uma paciente com anemia crônica causada pela deficiência de ferro. Materiais e métodos: Estudo realizado por meio de coleta de dados do prontuário hospitalar da paciente. Relato de caso: Paciente E.F.S., sexo feminino, 61 anos, apresenta anemia crônica desde 2010, fazendo transfusões frequentes. A mesma deu entrada no hospital apresentando hemoglobina de 3,1 g/dL, ferritina 4,5 ng/mL e ferro sérico 31 ug/dL. Resultados/Discussão: Para investigar o caso da paciente, solicitou-se exames de rastreio de anemia e avaliação hematológica. Devido a hemoglobina baixa, a mesma recebeu transfusão de 2CH e reposição de ferro. Após a transfusão sanguínea, a hemoglobina aumentou para 5,3 g/dL. Ao decorrer do tratamento a paciente recebeu mais uma transfusão e permaneceu com a reposição de ferro, logo apresentou-se estável hemodinamicamente e com melhora dos sintomas, apresentando a hemoglobina com valor de 6,1 g/dL. Em avaliação hematológica, devido aos riscos de mais transfusões, optou-se em manter o tratamento com reposição de ferro endovenoso na UBS de origem e retornar ao ambulatório em duas semanas para avaliar novo perfil de ferro. Porém, a paciente voltou ao acompanhamento ambulatorial após 30 dias, tendo o resultado de hemoglobina 7,1 g/dL, ferro sérico 16,0 ug/dL, ferritina 15,9 ng/mL e capacidade de ligação de ferro 364,0 ug/dL. A concentração de hemoglobina considerada normal para mulheres é de 12 g/dL. Quanto aos valores de referência para ferro sérico, considera-se dentro dos níveis aceitáveis os valores de 37 a 170 ug/dL e para ferritina 4,6 a 204,0 ng/dL. Considerando o quadro da paciente, pode-se classificá-lo como anemia grave, pois a hemoglobina apresenta-se < 7 g/dL, consequentemente nesses casos indica-se a transfusão sanguínea. Entretanto, a transfusão ameniza o problema temporariamente, porém não corrige a anemia, a qual deve ser investigada a causa que está levando a deficiência de ferro, e investigar um possível sangramento crônico, para enfim tratar definitivamente a Anemia Ferropriva. Portanto, após estabelecer o paciente com a transfusão, deve-se continuar a reposição de ferro e acompanhar a evolução. Conclusão: Levando em consideração o relato de caso, percebe-se que houve melhora nos resultados finais da paciente, porém ainda não estão dentro dos valores de referências. Avaliando os relatos iniciais e a idade da paciente, nota-se que o caso clínico necessita de uma investigação mais criteriosa para obter resultados positivos com o tratamento.
