Diagnostic and prognostic value of heparin-binding protein in infection and critically ill adult patients: A systematic review and meta-analysis Article Swipe

Purpose: In this study, we seek to evaluate the diagnostic value of Heparin-binding protein (HBP) levels in infection and intended to investigate the prognostic value of HBP in critically ill adult patients. Methods:We searched PubMed, Web of science, EMBASE and the Cochrane Database to identify relevant studies on June 2023. We included studies on adult patients with infection or critically ill that reported HBP levels and the data required to create 2x2 tables. We used R software, Revman 5.4 and Stats/MP 17.0 to analyze the pooled diagnostic and prognostic accuracy outcomes. Quality Assessment of Diagnostic Accuracy Studies 2 tool (QUADAS-2) was used to assess the risk of bias of included studies. This study is registered with PROSPERO (CRD42023421081). Results: Fifty-six studies with 11486 patients were identified in our present study. Most included studies demonstrated a strong correlation between HBP levels and infection and severity of disease. HBP had a pooled sensitivity of 0.87 (95% CI, 0.82–0.91), a pooled specificity of 0.87 (95% CI, 0.79–0.92) and an AUC of 0.93 (95% CI, 0.91–0.95) for the diagnosis of infection. Otherwise, HBP had a pooled sensitivity of 0.77 (95% CI, 0.74–0.80), a pooled specificity of 0.72 (95% CI, 0.68–0.76) and an AUC of 0.81 (95% CI, 0.78–0.85) for the prognosis of critically ill adult patients. We demonstrated that HBP has better diagnostic and prognostic role of infection and critically ill than Procalcitonin, CRP (C-reactive protein) and WBC (White Blood Cell) in adult patients. Conclusions: The diagnostic ability of HBP is beneficial for early recognition of infection, which showed both high sensitivity and specificity in infectious diseases. In addition, our study revealed that HBP plays role of detecting aggravation of critically diseases, including organ dysfunction and mortality.
