Drug Shortages in a Pediatric Stem Cell Transplantation Unit: A 5-Year Analysis of Incidences and Solutions Article Swipe

Background: Drug shortages cause numerous difficulties for clinicians, pharmacists, patients and healthcare authorities worldwide. The last decade, different therapeutic classes essential during conditioning regimens and supportive care in hematopoietic stem cell transplantation (HSCT) have been affected by substantial shortages. Methodology: Incidence of drug shortages (July 2011-July 2016) in the pediatric HSCT unit (Ghent University Hospital, Belgium) as well as problem solving strategies were identified and analyzed. Drug shortages were defined as any breach (temporary/permanent) in supply of a pharmaceutical product involving either manufacturing problems or deviations in distribution. Results: A total of 84 individual shortages, affecting 22 different drugs (81.0% chemotherapeutics and 19.0% supportive care drugs) were detected. In total, 5164 shortage days were registered with a mean duration of 186 days (SD 246.5) per individual drug shortage. When combining all shortage periods trimethoprim/sulphamethoxazole, benzylpenicilline, ranitidine (supportive care) and melphalan, methotrexate (chemotherapeutics) had the longest supply disruptions. Solutions were purchase of a generic alternative or other formulation (36.9%), close follow-up of available stock in pharmacy to bridge the shortage (16.7%), urgent import from another (European) country (16.7%), search for an alternative provider (10.7%), pending derogation to be finalized (4.8%), delivery by specified quota (4.8%), modification of licensed drugs (2.4%), change of on-going treatment (1.2%) and cohorting of patients (1.2%). In 4.8%, the problem solving strategy was not documented. A persistently high number of drug shortages (average of 16.8 shortages per year) was observed. Discussion and Conclusion: This study measured the incidence of drug shortages in a pediatric HSCT department in a Belgian university hospital and reports the problem solving strategies applied. Proactive management by hospital pharmacists, close follow-up and increased pharmacist efforts, enabled to manage these drug shortages. However, the potential widespread impact of these shortages across institutions nationwide highlights the importance of a coordinated (inter)national effort to prevent drug shortages, especially in vulnerable population groups.
