Efficacy of the Stag-Beetle knife GX for endoscopic submucosal dissection in patient with gastric superficial neoplasms Article Swipe

胃粘膜下層剥離術（ESD）は2006年に保険収載されて以降，急速に普及してきた．それに伴いESD専用ナイフもこれまで多数開発されている．2016年6月にハサミ型ESDナイフであるSBナイフGXタイプ（SB-GX）が市販され，当院にも同年9月に導入した．そこでSB-GX使用の有無での当院における治療成績を検討した．2014年4月から2017年8月まで当院で施行した胃ESD 168病変を対象とした．除外症例56病変を除いた112病変の内，SB-GXを先発使用した21病変をGX群，デュアルナイフを先発使用した91病変をDual群として処置時間，一括切除率，後出血率，穿孔率について後ろ向きに比較検討を行った．いずれも有意差は認めないもののGX群では後出血を1例も認めなかった．また処置時間は両群間で同等であった．次に両群間の背景因子を揃えるため傾向スコアによるマッチングを行った．マッチング後の解析では，有意差はないもののGX群の処置時間が短い傾向を示した一方で有害事象の増加は認めなかった．今回の検討ではSB-GXの有用性は立証できなかったが，後出血率低下，処置時間短縮に寄与する可能性が示唆された．今後さらに症例を積み重ねたうえでの再検討が望まれる．
