Traditional Fault Tree Analysis (FTA), known Kinetic Tree Theory (KTT), was derived by Vesely [1] in the 1970s to model and analyse engineering systems.The tree provides a clear visual representation of the causes of system failure in terms of component and failures and human errors.FTA has 2 stages, analysis which involves identifying the necessary and sufficient causes of system failure, i.e., the minimal cut sets and quantitative analysis which involves calculating the system unavailability, system failure frequency and measures of importance.
