El Hiperparatiroidismo primario (HPTP) es un \ntrastorno endocrino frecuente caracterizado \npor una secreción autónoma de hormona \nparatiroidea (PTH). Si bien, su variante asintomática es la \nmás frecuentemente encontrada en la práctica clínica (la \ncual se diagnostica incidentalmente), las complicaciones \nóseas y renales afectan de forma importante la calidad \nde vida del paciente. Sin embargo, el espectro de manifestaciones \nde este trastorno no se limita al metabolismo \nmineral, puesto que las concentraciones elevadas de PTH \nse asocian a un mayor riesgo de alteraciones metabólicas \ncomo el síndrome metabólico, diabetes mellitus 2 y enfermedades \ncardiovasculares. En este ámbito, la hipertensión \narterial (HTA), relacionada con aproximadamente 9,4 millones \nde muerte al año, ha sido considerada una manifestación \nno clásica del HPTP. La interacción PTH-Aldosterona \nha surgido como un importante eslabón para tratar de \nexplicar esta relación, planteándose diversos mecanismos \nteóricos que posicionan a la PTH como estimulador directo \nde la síntesis de aldosterona en las células de la zona \nglomerular. Sin embargo, estos mecanismos teóricos han \nestado rodeados de controversia en su aspecto epidemiológico \ny clínico, existiendo aún muy pocos estudios poblacionales \nexplorando este vínculo y su relación con la \nmorbimortalidad cardiovascular, por lo que es necesario \nmayor investigación en el área con el fin de conocer el \nverdadero impacto de estos mecanismos en la salud de los \nindividuos. Esta revisión resume aspectos del metabolismo \ndel calcio, al igual que los principales mecanismos subyacentes \nal vínculo HPTP-HTA, y los datos epidemiológicos \ndisponibles sobre el tópico, a fin de brindar un mejor entendimiento \nsobre este novel planteamiento.
