Introduction: Leprosy reaction is an acute inflammatory episode as a complication of Mycobacterium leprae infection. The development of leprosy reaction is also influenced by genetic traits. Gene polymorphisms have been implicated in the development of leprosy. In terms of phenotype, single nucleotide polymorphisms (SNPs) of interleukin 6 (IL-6) are associated with the incidence of leprosy type 2 reactions. For this reason, the purpose of this study was to determine the relationship between the SNP of the IL-6 gene rs2069845 and the incidence of type 2 leprosy reactions in leprosy patients at Prof. Dr. I.G.N.G Ngoerah Hospital. Methods: This study is an observational study with a cross-sectional research design. Researchers will measure variables at one specific time, without follow-up. The sampling technique in this study used consecutive sampling technique. The sample consists of two groups: leprosy patients with type 2 leprosy reactions, and families of patients who do not have leprosy. Both groups will be tested for IL-6 SNPs using the PCR procedure. Data were analyzed using SPSS. Results: Based on the results of this study, the total sample obtained was 38 people with 18 people in the case group and 20 people in the control group. The average age of MH patients was 39.56 ± 17.23 years. Bivariate analysis found a significant association between ENL reaction and IL-6 gene SNP rs2069845 (p=0.020; IK95%=0.038 - 0.820). Multivariate analysis found that patients with IL-6 gene rs2069845 SNP can significantly increase the risk of ENL in MH patients by 5.6 times greater than patients without SNP (p value=0.027, IK95%= 1.219-36.334). Conclusion: There is an association between IL-6 gene SNP rs2069845 polymorphism and the incidence of type 2 leprosy reaction in leprosy patients. Pendahuluan: Reaksi kusta merupakan episode inflamasi akut sebagai komplikasi dari infeksi Mycobacterium leprae. Perkembangan reaksi kusta juga dipengaruhi oleh sifat genetik. Polimorfisme gen diteliti telah menjadi penyebab dari perkembangan kusta. Ditinjau dari fenotipnya, single nucleotide polymorphisms (SNP) interleukin 6 (IL-6) berkaitan dengan kejadian reaksi kusta tipe 2. Untuk itu tujuan dari penelitian ini adalah untuk mengetahui hubungan antara SNP gen IL-6 rs2069845 terhadap kejadian reaksi kusta tipe 2 pada pasien kusta di RSUP Prof. dr. I.G.N.G Ngoerah. Metode: Penelitian ini merupakan penelitian observasional dengan desain penelitian cross-sectional yaitu. Peneliti akan melakukan pengukuran variabel pada satu saat tertentu, tanpa adanya follow-up. Teknik pengambilan sampel dalam penelitian ini menggunakan teknik consecutive sampling. Sampel terdiri dari dua kelompok yaitu pasien kusta dengan reaksi kusta tipe 2, dan keluarga pasien yang tidak mengalami kusta. Kedua kelompok tersebut akan dilakukan pemeriksaan SNP IL-6 menggunakan prosedur PCR. Data dianalisis menggunakan SPSS. Hasil: Berdasarkan hasil penelitian ini total sampel yang diperoleh sebesar 38 orang dengan jumlah 18 orang pada kelompok kasus dan 20 orang pada kelompok kontrol. Usia rata-rata pada pasien MH yaitu 39,56±17,23 tahun. Analisis bivariat didapatkan hubungan yang signifikan antara reaksi ENL terhadap SNP Gen IL-6 rs2069845 (p=0,020; IK95%=0,038 – 0,820). Analisis multivariat menemukan bahwa pasien dengan SNP Gen IL-6 rs2069845 dapat secara signifikan meningkatkan risiko kejadian ENL pada pasien MH sebesar 5,6 kali lebih besar dibandingkan dengan pasien tanpa SNP (nilai p=0,027, IK95%= 1,219-36,334). Simpulan: Terdapat hubungan antara polimorfisme SNP gen IL-6 rs2069845 terhadap kejadian reaksi kusta tipe 2 pada pasien kusta.
