Identifying the Distinct Mental Health Treatment Needs of Transgender and Nonbinary Youth Article Swipe

Transgender and nonbinary youth (TNBY) face a disproportionately high risk for mental health problems and barriers to receiving appropriate mental health care. For example, most mental health providers lack training in how to treat TNBY, despite the existence of clinical guidelines. However, most existing guidelines were developed by clinicians and may overlook the needs of the target community: TNBY.Our qualitative study addressed this gap by analyzing data from separate focus groups comprised of TNBY (18-23 years; N=6) and expert mental health providers (N=11) about therapeutic practices for working with TNBY. Each group met three times to discuss critical therapy practices for TNBY and to review a list of best practices identified in a previously conducted literature review. Transcripts were inductively and deductively coded and thematically analyzed. Analysis focused on recommendations specific to TNBY distinct from existing literature and provider recommendations. TNBY recommendations fell under three broad themes: 1) Expect to make mistakes and elicit TNBY input accordingly (e.g., ask for client input about the language you use, whether you have been asking invasive questions); 2) Research TNBY-related novel topics independently between sessions and refrain from asking clients to educate you (e.g., researching policies that will affect TNBY clients); 3) Tailor resource recommendations to the needs of TNBY (e.g., recognize that typical inpatient services and crisis support lines can be transphobic). These practices go beyond what is currently considered best practices as captured in the literature by expert providers surveyed in this study.
