Indications et Résultats de la Cœlioscopie Diagnostique à l’Hôpital National de Niamey : Une Étude Rétrospective de 65 Patients Article Swipe

RESUMEIntroduction. L’objectif de ce travail etait decrire notre activite de cœlioscopie dans l’approche diagnostique a l’hopital National de Niamey. Patients et methodes. Il s’agissait d’une etude transversale retrospective descriptive sur une periode de dix ans allant de janvier 2006 a decembre 2015. Nous avons inclus tous les patients ayant beneficie d’une cœlioscopie d’abord a visee diagnostique durant la periode de l’etude. Les parametres analyses etaient les donnees demographiques, l’indication de la cœlioscopie, le diagnostic operatoire et le taux de conversion en laparotomie. Resultats. Durant la periode de l’etude, 20 642 patients ont ete operes en chirurgie viscerale dont 447 (14,54 %) par cœlioscopie. Parmi eux 65 patients (0,31%) ont eu une cœlioscopie diagnostique et sont evalues dans ce travail. Leur âge moyen des patients etait de 35,9 ans (± 9,2 ans) avec des extremes allant de 16 ans a 72 ans. Le pic de frequence se situait dans la tranche d’âge comprise entre 31 ans a 45 ans (33 cas soit 50,76 %). 76,94 % des patientes etait de sexe feminin. La douleur abdomino-pelvienne etait le motif d’admission le plus frequent ( 78.46 %). La douleur abdomino-pelvienne chronique etait l’indication la plus frequente (64,63 %). Les diagnostics peroperatoires les plus frequents etait les adherences multiples trouvees chez 28 patients (43,0%), les kystes ovariens rompus dans 8 cas (12,30%), l’hypogenesie ou agenesie des organes genitaux internes, l’endometriose et l’epanchement peritoneal dans 5 cas (7,69 %). L’adhesiolyse a ete le geste le plus realise. La conversion en laparotomie a ete effectuee chez 4 patients (6,15%). Conclusion. La cœlioscopie est une methode fiable pour poser le diagnostic ; elle a aussi une valeur therapeutique chez certains patients.ABSTRACTIntroduction. The objective of this work was to report our activity of diagnostic laparoscopy at the National Hospital of Niamey. Patients and methods. This was a cross-sectional descriptive retrospective study over a ten-year period from January 2006 to December 2015. Our study population was made of all patients who underwent laparoscopy for diagnostic purpose during the period of study. Our study variables were demographic data, indication of the laparoscopy, operative findings and conversion rate to laparotomy. Results. During the study period, 20642 patients had visceral surgery operations. 447 patients had laparoscopy (14.54%) and among them 65 patients (0.31%) had diagnostic laparoscopy and are reported in this study. Their average age was 35.9 years (± 9.2 years) with extremes ranging from 16 to 72 years. 33 patients (50.76%) were in the age group between 31 and 45 years. 76.94% of patients were women (sex ratio H/F= 0.3). Abdominal-pelvic pain was the most common reason for admitting patients (78.46%). Chronic abdominopelvic pain was also the most common indication for coeliodiagnosis (64.63%). The most common operative findings were multiple adhesions (28 patients, 43.0%), ruptured ovarian cysts (8 patients, 12.30%); hypogenesis or agenesis of the internal genital organs, endometriosis and peritoneal effusion (5 patients, 7.69% each). Adhesiolysis was the most performed operative act. Conversion to laparotomy was done in 4 patients (6.15%). Conclusion. Laparoscopy is a reliable method for diagnosis at Niamey. It may also have a therapeutic value in many patients.
