Mesorectal Hemorrhage and Rectal Perforation in a Patient with Intestinal Amyloidosis—A Case Report— Article Swipe

腸管アミロイドーシスは，血管壁や腸管壁へのアミロイド蛋白の沈着により組織の脆弱性をきたす稀な病態である．症例は66歳，男性．突然の下腹部痛と下血を主訴に当院へ救急搬送され，直腸間膜内出血の診断で緊急手術とした．術中所見では，直腸間膜内血腫と直腸Rb後壁に約3cmの裂創を認め，裂創部と血腫を含めたHartmann手術を施行した．摘出標本は肉眼的に正常粘膜を呈し，病理組織学的所見では腸管全層の血管壁にアミロイド沈着を認め，腸管アミロイドーシスと診断した．本症例では，脆弱性をきたした間膜内血管が破綻して血腫形成・直腸穿孔に至ったと考えられた．腸管アミロイドーシスは腸間膜出血のリスクになるとされるが，海外の文献も含めて過去の報告は小腸間膜出血の2例のみだった．原因不明の腸間膜内出血や穿孔では，腸管アミロイドーシスも鑑別に含み診療にあたるべきである．
