Aim: Double gall bladder, with or without duplication of the cystic duct, is a rare congenital anomaly and is challenging to surgeons due to the unusual anatomy and increased risk of post-operative complications. In this operative video, robotic assisted laparoscopic cholecystectomy was demonstrated. Methods: A 68-year-old male underwent CT abdomen for investigation of right sided abdominal pain. The CT discovered double gallbladder with gallstones.MRCP showed complete gallbladder duplication with two cystic ducts entering the common bile duct. The patient proceeded to elective robotic assisted laparoscopic cholecystectomy. Two gallbladders were identified, each with its own cystic duct. However, there was only a single cystic artery. Results: The operation time was 140 min. There were no post-operative complications and the patient was discharged on the first operative day.Pathology showed two gallbladders with features of chronic cholecystitis. The patient remained pain-free at surgical follow-up.Conclusion: Double gallbladder is a rare anatomical variant that is seldom detected at pre-operative imaging.Minimal access cholecystectomy can be performed safely for patients with double gallbladder.
