Introduction: This case report presents an innovative non-surgical treatment strategy for a complex case of skeletal Class III malocclusion accompanied by multiple congenitally missing teeth, demonstrating the effectiveness of combined orthodontic-prosthetic intervention. Case description: A 21-year-old male patient presented with characteristic skeletal Class III features including anterior open bite, concave facial profile, and absence of nine permanent teeth. Comprehensive evaluation revealed inadequate alveolar bone for implant placement. The treatment protocol incorporated: 1) digital orthodontic alignment for optimal space redistribution, 2) mandibular fixed bridge prostheses for functional restoration, and 3) a maxillary double-denture design to address the skeletal discrepancy. Discussion: The implemented treatment achieved significant improvements in both occlusal function and facial aesthetics. Functional occlusal contacts increased from 8 to full-arch, while the maxillary double-denture effectively compensated for the skeletal imbalance. The digital workflow enhanced treatment precision and predictability. Importantly, this conservative approach preserved the option for future surgical intervention while delivering immediate clinical benefits. Conclusion/clinical significance: This case illustrates that a carefully planned multidisciplinary approach can successfully manage complex skeletal malocclusions without surgical intervention, offering a valuable alternative for patients declining orthognathic surgery. The integration of digital orthodontics with advanced prosthetic techniques proved particularly effective in achieving both functional and aesthetic rehabilitation.
