Vaginal pessary has been used for treating pelvic organ prolapse. It is common, easy and safe to use. However, failure to retain the pessary is a common problem. Levator ani muscle (LAM) injury had proved to be associated with pelvic organ prolapse and treatment outcome (1,2). This study aims to investigate any association between LAM injury and the successfulness to retain a vaginal pessary in symptomatic pelvic organ prolapse women. This is a secondary analysis of a randomised controlled trial. Women were recruited from Nov 2011 to Nov 2013, with symptomatic pelvic organ prolapse (Stage I to Stage III) and received no previous treatment. They are randomised to have vaginal ring pessary or conservative treatment. Pelvic floor exercise was taught and encouraged in both groups. Secondary analysis was performed according to the final treatment received. Translabial 3DUS was performed at rest and pelvic floor contraction (PFMC) at baseline assessment. Offline analysis was performed to LAM injury. LAM injury was defined using a cut-off of 23.6 mm for levator-urethral gap in at least three central slides obtained at the level of minimal hiatal dimension, 2.5 and 5 mm above at US volumes obtained at PFMC. 276 women were recruited for the study, 137 of them were randomised for conservative treatment while 139 of them were randomised for vaginal pessary with similar baseline characteristics including age, parity, body mass index and stage of prolapse. After 12 months, 83 women remained well with the vaginal pessary in the pessary group while 41 of them had pessary dropped out despite at least 2 further attempts. LAM injury (including unilateral or bilateral) was identified in 28 (33.7%) and 19 (46.3%) of them. There was no significant difference among those succeed or failed to retain pessary with p value = 0.17. Levator muscle injury was not associated with successful fitting of vaginal pessary in women with pelvic organ prolapse.
