Despite excellent continence rates (80%) and patient satisfaction (90%), AUS implants are not without complications. Approximately 26% will eventually undergo reoperation. Our objective was to characterize the variation and frequency of microbial growth on AUS devices undergoing explantation. Additionally, we sought to identify patterns of colonization and compare them to existing data on colonization of explanted penile prosthesis in literature. METHODS This was a single institution, single surgeon retrospective study which examined 62 artificial sphincter (AUS) explants on 51 patients from 2008-2017. Data was collected regarding the surgical indications for explantation, as well as the culture results from the AUS implant. All explanted AUS devices were sent for culture, unless they were sent back to the manufacturer for defective investigation. Indications for explantation included revision, infection/erosion, and other. Culture results were then analyzed by indication. RESULTS A total of 62 AUS explant procedures were performed, including 39 for revisions, 21 for erosions/ infections, and 2 for other reasons. 13 (21%) of the AUS explants were sent back to the manufacturer for inspection and not submitted for culture. Of the 49 AUS explants submitted for culture, 19 (39%) had no growth. Of the remaining 30 explants, coagulase negative Staphylococcus spp. was the most commonly found organism (50%), followed by Propionibacterium Acnes (37%), and Enterococcus Faecalis (20%). Over half of the AUS explants had multiple organisms found on culture (53%). The most commonly isolated organisms for erosion/ infection were coagulase negative Staphylococcus, while the most common organism found on revisions was Propionibacterium. The infected/eroded explants contained more urinary pathogens (E. Coli, Proteus Mirabilis, Staphylococcus Aureus, Pseudomonas) than revision or other explants. Candida was isolated in 4 explants (13%) and all were infected/eroded. CONCLUSIONS While the majority of AUS explants were not performed due to active infection, a significant portion of these were colonized with bacteria sufficient for culture. The most common organisms were comprised of typical skin flora, and over half grew multiple types of bacteria.
