Zusammenfassung Seit nunmehr 10 Jahren stehen Medikamente auf der Basis anorektischer Peptidhormone zur Behandlung der Adipositas (OMM) zur Verfügung. In dieser Übersichtsarbeit wird die Evidenz zu den 3 behördlich zugelassenen und am häufigsten verwendeten Medikamenten (Liraglutid, Semaglutid und Tirzepatid) dargestellt. In diesem Kontext wurde innerhalb des SCALE-Programms (Liraglutid), des STEP-Programm (Semaglutid) und des SURMOUNT-Programms (Tirzepatid) eine Vielzahl von hochwertigen placebokontrollierten prospektiv randomisierten Studien durchgeführt. Demnach liegt der Wirkungsgrad auf die mittlere Gewichtsabnahme für Liraglutid bei 6–8 %, für Semaglutid bei 9,6–15 % und für Tirzepatid bei 14,7–22,1 %. Ebenso zeigen die Medikamente einen hohen antidiabetischen Wirkungsgrad, wobei die erreichte Gewichtsabnahme bei Diabetikern meist geringer ist. Insgesamt sind die Sicherheit und Verträglichkeit der 3 Medikamente hoch. Die häufigsten unerwünschten Ereignisse treten meist früh während der Behandlung auf und klingen mit der Zeit ab. Die positiven Effekte werden durch ein Absetzen der Therapie meist nivelliert. Bezüglich harter Endpunkte konnte die SELECT-Studie für schwere unerwünschte kardiovaskuläre Ereignisse (MACE) eine Risikoreduktion von 20 % unter Semaglutid zeigen. Daten zu weiteren harten Endpunkten und zum Langzeitverlauf existieren bisher nicht.
