Placental Senescence In utero: Molecular Markers, Imaging Correlates, and the Next Frontier in Perinatal Risk Stratification Article Swipe

Despite advances in prenatal care, conditions such as late stillbirth, intrauterine growth restriction, and pre-eclampsia continue to defy early detection, often occurring in pregnancies previously considered low risk. Emerging evidence suggests a common underlying mechanism: premature placental senescence that unfolds in utero and independently of gestational age. However, current diagnostic tools lack the capacity to detect this biological aging process in real time. This review synthesizes the latest molecular, imaging, and pathological data to map the trajectory of placental aging before birth. Key biomarkers – including p16 INK4a overexpression, telomere attrition, mitochondrial dysfunction, and epigenetic age acceleration – are examined alongside advanced imaging modalities such as Doppler velocimetry, functional magnetic resonance imaging, and placental radiomics. Histopathologic correlates further contextualize these findings. Through a structured literature review guided by PRISMA principles, we propose a conceptual framework for estimating biological placental age using integrated molecular and imaging data. By reframing placental aging as a dynamic, measurable, and potentially modifiable process, this work advocates for a paradigm shift in obstetric care – from reactive management to predictive monitoring. Advancing senescence-based diagnostics may enable earlier intervention and improve outcomes in pregnancies previously vulnerable to undetected decline.
