Aim: to present 10-year experience results in choroidal melanoma (CM) management by primary endoresection (ER). Materials and Methods: 35 patients (35 eyes) with CM aged from 30 to 75 years (mean — 58 years) were treated by the primary ER method from 2014 to 2024. There were 9 (26%) women and 26 (74%) men among them. According to the international TNM classification (AJCC) (2017), 11 (31%) and 24 (69%) patients had CMs of T3 and T2 stages, respectively. No CM metastases were detected in any patient at the time of treatment. Before the treatment, the tumor thickness ranged from 6.0 to 11.6 mm (mean — 8.2 mm), and its basal diameter ranged from 7.8 to 13.9 mm (mean — 11 mm). The average follow-up was 69 months. Results: histological examination of the ER-related MC specimens revealed two variants of melanocyte phenotypes: spindle cell (23 cases) and mixed epithelioid-spindle cell (12 cases) ones. After the treatment, the best corrected visual acuity ranged from light perception with incorrect light projection to 0.8 (mean — 0.15). In a long-term follow-up (3 — 18 months, mean — 73 months), complications were reported in 62% of patients. In a long-term follow-up (3 — 18 months), complications were reported in 62% of patients. Posterior subcapsular cataract was the most common complication (n=15, 43%). In 91% of cases (n=32), the treated eye was preserved. Conclusion: our long-term experience of CM endoresection proves the efficacy, safety and feasibility of wider application of the method preventing enucleation without increasing a risk of metastasis in patients with "large" CMs. KEYWORDS: uveal melanoma, primary endoresection, vitreoretinal surgery, choroidal melanoma, large tumors.
