Background: There are conflicting reports on the association between vitamin D status and inflammatory markers in patients with rheumatoid arthritis.Some studies have indicated that vitamin D deficiency may be associated with increased inflammatory response while others have failed to show this trend.Methods: In this retrospective study we analysed the impact of vitamin D deficiency on inflammatory markers in a cohort of patients with rheumatoid arthritis in west London.All patients admitted to hospital or had an acute infection were excluded from the study.Vitamin D deficiency was defined as total 25 hydroxy-vitamin D level less than 30 nmol/L, level between 30 and 79 were defined as insufficient and levels more than 80 were regarded as normal.Results: 185 patients (116 white, 44 south Asian, 25 black) were analysed.The majority of patients had insufficient vitamin D levels.There was no association between inflammatory markers (CRP and ESR) and vitamin D status in patients with rheumatoid arthritis.However, there was a significant association between age and vitamin D levels.Younger patients had significantly lower vitamin D levels compared with older patients.This may be explained in part by the fact that older patients are more likely to be assessed for osteoporosis and therefore more likely to be on vitamin D replacement.There are a number of other possibilities however.Vitamin D deficiency is more common in ethnicities with darker skin tones (skin type III to VI).It is possible that the younger population represent a more ethnically diverse cohort with a significant proportion with darker skin types.Vitamin D deficiency has also been linked to obesity.The rise in obesity may be more prevalent in the younger populations.The skin protection programmes devised with a view to reducing the incidence of skin cancer have been targeted predominantly at the young.The use of sun screen can completely obliterate the ability of solar UV to form vitamin D in the skin.Behaviours and practices which impede vitamin synthesis may be more common amongst the young.Conclusion: Vitamin D status in our population of rheumatoid arthritis patients was not associated with raised inflammatory markers, however there was a significant association between age and vitamin D levels.This conclusion needs to be validated in large multicentre study with large number of patients.
