Orbital cellulitis or post-septal cellulitis is a severe infection that can result in vision loss or life-threatening complications if not treated promptly. Intravenous antibiotics are the standard therapy, but the use of adjunctive steroids to reduce inflammation has been increasingly considered, though it remains controversial. This case report describes a 40-year-old female with orbital cellulitis who was initially treated with intravenous antibiotics but showed limited improvement. Following the addition of systemic steroids, the patient exhibited rapid clinical and pathological improvement. The case highlights the potential role of steroids in accelerating recovery by reducing orbital inflammation once infection control is established. Although steroids may enhance symptom resolution and reduce complications like ophthalmoplegia, they must be used cautiously after adequate antibiotic therapy to avoid masking residual infection. This report adds to the growing evidence supporting the judicious use of steroids in managing orbital cellulitis alongside antibiotic therapy.
