Santé sexuelle et reproductive des femmes migrantes sans logement hébergées à l’hôtel en Ile-de-France Article Swipe
In the Ile-de-France region a growing number of homeless migrant women call on the emergency housing system for shelter. They are housed with their children in hotels scattered across the region. Because of barriers to access personal housing, this temporary housing option often last several years. This group of women encounter several types of vulnerability : housing instability, economic hearship and precarious administrative status. This suggests poor health status and limited access to healthcare, specifically sexual and reproductive health care. We investigate here various aspects of sexual and reproductive health in this group : the conditions of access to health services, access to cervical cancer screening and HIV test, contraception use and elements of perinatal health. We analyzed data from the DSAFHIR survey conducted in 2017 : 469 migrant women housed in 15 hotels participated. Our results suggested that medical insurance coverage strongly predicted health care utilization, and poor access to general practitionners. We found low uptake of cervical cancer screening. Like HIV test, it was associated with educational level and French language proficiency. It appeared that women in this group were likely to be tested for HIV at the beginning of their stay in France, while cervical cancer screening occured later. Compared to levels in the French general population, contraception use was low, with a high use of the contraceptive implant. Regarding perinatal health, and compared to women born in France, women who were born abroad had increased risk of inadequat gestational weight gain. However, within this group, there was no increased risk for women with unstable housing. Our analyses confirmed a high risk of cesarean section among women born in Sub Saharan Africa. Moreover, we found that among multiparous women with no prior cesarean sections, risk of cesarean section was highest in the first birth in France, and was lower in subsequent births.
