58歳女性.35歳で腎ドナーとして右腎を摘出した.その後,婦人科疾患術後の癒着性イレウスのため,回腸瘻およびS状結腸瘻を造設された.回腸瘻の排液が過剰なため,容易に脱水症となり入退院を繰り返した.頻回の点滴治療で四肢の末梢静脈が荒廃し,中心静脈カテーテルや皮下植込み型中心静脈ポートを留置するも,カテーテル感染を度々起こした.脱水による急性腎不全を繰り返し末期腎不全に至った.X年,自己血管での血液透析を希望され,エコーガイド下内頸静脈直接穿刺法による単針透析で治療を開始した.透析効率を向上させるため動脈表在化を作製し,上腕動脈から脱血を行い,内頸静脈へ返血し透析を継続している.本症例はバスキュラーアクセスの選択に苦慮した維持透析患者であるが,内頸静脈直接穿刺法はエコーを使用し安全に実施可能であり,末梢血管が荒廃したりカテーテル感染が懸念されたりする症例では,有用な方法である.
