Subscapularis healing has been demonstrated to be critical in postoperative outcomes after total shoulder arthroplasty. There have been several proposed subscapularis management techniques, including tenotomy, peeling, and lesser tuberosity osteotomy. However, there is some controversy regarding the best approach to the subscapularis. The purpose of this chapter is to provide an overview of subscapularis anatomy, approach, repair techniques, and clinical outcomes in the setting of total shoulder arthroplasty.
