背景.肺多形癌は悪性度の高い稀な腫瘍で,化学療法抵抗性として知られる.Pembrolizumabによる治療を行った肺多形癌の2例を経験したので報告する.症例1.64歳,男性.右背部痛を主訴に受診し,CTで右肺下葉に腫瘤と胸膜結節を指摘された.腫瘤に対しCTガイド下経皮生検を行い肺多形癌(cT4N0M1a,stage IVA)の診断が得られた.PD-L1の高発現(tumor proportion score 100%)を認め,pembrolizumabによる治療を行ったところ腫瘍は縮小した.症例2.62歳,女性.外科的手術後,扁平上皮癌と紡錘形の肉腫様成分からなる肺多形癌(pT3N0M0,stage IIB)と診断された.PD-L1は低発現(tumor proportion score 30%)であった.Carboplatin及びnab-paclitaxelによる一次治療を行い奏効したのちに再発し,二次治療としてpembrolizumabを投与したが,腫瘍は増大し奏効しなかった.結論.PD-L1高発現の肺多形癌に対してpembrolizumabが有効である可能性が示唆された.
