Introduction: Emergency Departments are under considerable pressure, with the number of attendances increasing alongside a considerable backlog of patients waiting for treatment. Across England, A&E attendances and emergency admissions in October 2023 were 1.4% higher than the same period the previous year.1 These pressures exacerbate existing inequalities as people from more deprived backgrounds wait longer, and those from ethnic minority backgrounds are more likely to access care late.2 The aim of this project was to understand what inequalities existed in use and access to the Emergency Department at Southampton General Hospital. Materials and methods: This study conducts analysis of cross-sectional data from 139,356 walk-ins from 98,726 independent patients to Southampton General Hospital Emergency Department (ED) between 1 January 2022 and 30 April 2023. Demographic data was analysed alongside ED specific coding for attendance reason, referral source, Emergency Care Data Set (ECDS) codes, discharge diagnosis, admission flags and pre-existing conditions using the Charlson Comorbidity Index. Data analysis was carried out in Excel. Results and discussion: Almost a third of all walk-ins to the Emergency Department (28%) were aged 0–15 and attendance was similar across each of the deprivation deciles and comparable to the local population. 33% of all walk-ins were from patients registered with 10 GP practices in the Southampton area, and 6 of the 10 practices were in the 20% most deprived areas. 32% of walk-ins were referred by a GP or healthcare provider (n= 44,750). Ethnic groups other than White British and those from more deprived areas were less likely to be referred to the Emergency Department by a GP or healthcare provider. When comparing to local population data, women from Black or Asian backgrounds were more likely to present to the Emergency Department with pregnancy related problems than women from other ethnic backgrounds. Analysis showed no relationship between Charlson Comorbidity Index score and number of ED visits. No link was noted between Charlson score and IMD Decile or the probability of being admitted. Conclusion: In times of increased pressure across primary and secondary care, it is important to consider whether patients are attending the most appropriate site for their care and whether there are inequalities in access and outcomes. This project is a starting point to understanding who is accessing services in the Emergency Department at Southampton General Hospital. Work is now continuing with relevant departments in the hospital and with partners in the local area to gain further understanding of any inequalities that exist within the system.
