Valve-Sparing Root Replacement for Syphilitic Aortic Arch Aneurysm with Aortic Regurgitation Article Swipe

梅毒性大動脈瘤はペニシリンの登場以来，稀な疾患となった．梅毒性大動脈瘤は大動脈弁輪拡大を来し，大動脈弁閉鎖不全症を合併することが知られている．今回われわれは，大動脈弁閉鎖不全症を合併した梅毒性大動脈瘤に対して自己弁温存基部置換術および全弓部置換術を施行し良好な結果を得られたので報告する．症例は55歳男性．大腸憩室にて他院入院中に施行されたCTにて上行大動脈瘤を指摘され当院紹介となった．術前CTでは最大短径68 mmの上行大動脈～弓部大動脈瘤を認めた．心エコーでは大動脈弁輪拡大に伴う重症大動脈弁閉鎖不全症を認め，弁尖の変化は軽度であった．術前検査にて梅毒検査陽性であり，梅毒性大動脈瘤が疑われた．術中所見で大動脈弁弁尖の器質的変化を認めず自己弁温存が可能と判断し，reimplantation法による自己弁温存基部置換術＋全弓部置換術を施行し，合併症なく術後11日目に退院した．
