56歳，男性。15年前に円板状エリテマトーデスと診断されたが途中通院を自己中断し，その間外用・内服治療なく経過した。右手背部のdiscold lupus erythematosus（DLE）皮疹部を掻破し自壊，増殖性の結節が出現した。受診時に右膝の紅色局面を同時に指摘された。右手背の腫瘤と右膝の角化性結節を生検したところ，前者は有棘細胞癌，後者は疣贅様変化を認めた。右手背腫瘤を全切除し左前外側大腿皮弁にて再建した。右手背全切除標本にても組織学的に表皮索が舌状に増大し癌真珠を…
