Abstract Background The aim of this Delphi study was to reach consensus on a new clinical decision tool to help identify or exclude Crohn’s disease (CD) in patients with perianal fistula(s) (PAF). Methods A panel of international experts in the field of proctology/inflammatory bowel disease was invited to participate. In the first round (electronic survey), participants were asked to anonymously provide their opinion probing (1) the relevance and use of clinical characteristics suggestive of underlying CD, (2) the…

Abstract Background and Aims Diet may influence the disease course in inflammatory bowel disease, but its role in postoperative outcomes for Crohn’s disease (CD) remains unclear. This study aimed to assess the association of macro- and micronutrient intake with endoscopic postoperative recurrence (ePOR) in a prospective multicenter cohort. Methods Patients with CD following ileocolic resection were prospectively recruited from six North American centers. Primary study outcome was ePOR (modified Rutgeerts’ score ≥i…

Association Between Maternal IBD-Related Antibodies and Early Life Gut Inflammatory Status and Microbiota Composition: Insights from Cord Blood
2025-11-01 • Paola Nicoletti, María Manuela Estevinho, Leonid Tarassishin, Mellissa Picker, Anketse Debebe, Isabella Nguyen, Rhiannon Dutra-Clarke, James L. Sto...
Our findings suggest that maternal IBD-associated antibodies cross the placenta barrier and may be associated with intestinal inflammation and imbalanced microbiota colonization. Whether these serological profiles negatively influence the priming of the baby's immune system or IBD risk later in life remains to be determined.

Maternal IBD-related antibodies are associated with early life gut inflammatory status and microbiota composition: insights from cord blood
2025-08-06 • Tae Gyu Kim, Paola Nicoletti, María Manuela Estevinho, Leonid Tarassishin, Mellissa Picker, Anketse Debebe, Isabella Nguyen, Rhiannon Dutra-Clarke,...
Abstract Purpose Antibodies in peripheral blood are used to aid in the diagnosis of inflammatory bowel disease (IBD), but their presence in neonatal cord blood and potential effects on early life development remain unknown. Methods We measured anti-CBir1, ANCA, anti-OmpC, ASCA IgA, and ASCA IgG levels in the cord blood of babies born to 78 mothers with or without IBD. Their association with fecal calprotectin (FC), and microbiota composition, characterized by 16S rRNA sequencing, was assessed throughout pregnancy …

Relationship of Intestinal Ultrasound With Clinical, Biochemical, and Endoscopic Disease Severity in Acute Severe Ulcerative Colitis: A Blinded, Prospective Study

INTRODUCTION: The utility of intestinal ultrasound (IUS) for acute severe ulcerative colitis (ASUC) is understudied. We correlated IUS to clinical, biochemical, and endoscopic disease severity in ASUC. METHODS: In a blinded, prospective study of 20 ASUC subjects, we analyzed standard IUS parameters (bowel wall thickening, vascular flow, wall stratification) and UC IUS indices (International Bowel Ultrasound Segmental Activity Score (IBUS-SAS), Milan-UC, UC-IUS) alongside modified Mayo symptoms scores, C-reactive p…
