症例は60歳の男性.30年前に下垂体腫瘍に対して経蝶形骨洞的腫瘍摘出術が行われ,術中大量出血をきたした.今回,突然の大量鼻出血により,貧血となり当院に搬送となった.蝶形骨洞後壁の骨窓より内頚動脈C3部の小動脈瘤が露出しており,出血源と考えられた.その後,バイパス併用母血管閉塞術を行い,経過は良好である.経蝶形骨洞術後の内頚動脈損傷および遅発性仮性動脈瘤形成は非常に重篤な合併症である.文献的考察を交えて報告する.
