症例は70歳,男性。孤立性左内腸骨動脈瘤破裂による出血性ショックに対してコイル塞栓直後に開腹下に空置術を施行した。術後瘤の拡大があったためコイル塞栓を施行したが瘤が再拡大しため空置術後3年目に破裂時同様にオクリュージョンバルン(OB)を併用して後腹膜経路で瘤内へ流入する動脈の結紮と瘤縫縮を施行した。OBを併用することで術中の出血のコントロールができ再手術を安全に施行できた。
