69歳,女性。約10年前から右臀部に紅色結節がみられるも,自覚症状を伴わないため放置していた。約2年前から急激に増大したため,当科を受診した。初診時,右臀部に55mm×50mm大の角化を伴わない暗紫紅色腫瘍がみられた。腫瘤の近傍に約1cm大の紅色結節を伴っていた。身体学的所見として,右鼠径部のリンパ節の腫脹,右下腿の浮腫を認めた。病理組織学的には,表皮と連続性に比較的小型の異型を伴う類円形細胞の脂肪織への浸潤がみられたことから脈管浸潤を伴った悪性エクリン汗孔腫と診断した。全…
