症例は55歳の女性で, C型肝硬変および肝細胞癌と診断して, 肝動脈塞栓療法を6回繰り返した. 診断4年後に縮小していた腫瘍が再び増大し, 胸水, 腹水, 黄疸, 吐血および肝性脳症が出現し死亡した. 発熱はなく, 末梢血白血球数は52, 400 (好中球91.6%) /μlと増加した. 剖検では, 肝硬変に肝細胞癌 (多結節癒合型, 索状型, 中分化型) を合併し, 遠隔転移を認めた. 脾は250gで, 骨髄もとくに異常なかった. 血中granulocyte colony…

症例は21歳の女性.18歳の時定期検診でHBs抗原の陽性が判明したが,血清GOT, GPTが正常であったので経過観察中,21歳の時,吐気,嘔吐に続く食欲不振で発症し,GOT, 175KU, GPT 173KUの上昇に対し血清α-フェトプロテイン(AFP)は1,237ng/mlの高値を示した.その時点における生検肝組織で,壊死巣中ないし近接部位の残存肝細胞にAFP染色の陽性所見を得た.IFNβ 600～300×104単位28日間の投与でHBe抗原陽性からHBe抗体陽性へと転換…
