35歳，男性．63歳の母をドナーとする生体腎移植が施行された．ドナー腎にゼブラ小体の蓄積を認めたことから問診を追加し，幼少期の四肢疼痛の既往が明らかになった．酵素活性・遺伝子検索の結果，Fabry病ヘテロ接合体からヘミ接合体への生体腎移植であったことが判明した．両者とも術後腎機能は安定しており，酵素補充療法を開始した．若年男性における原因不明の腎不全の原疾患として，Fabry病を念頭に置く必要がある．
