症例は76歳の女性で,朝からの右季肋部痛と嘔気を主訴に近医を受診した.処方された内服薬を服用したが改善傾向がないため,当院に救急搬送された.腹部診察所見では右季肋部に圧痛を認めた.来院時検査所見では特記すべき異常所見を認めなかった.腹部造影CTでは胆囊は頸部と体部の移行部付近で狭窄しており,腫大した底部側胆囊では壁肥厚と造影効果の減弱を認めた.以上の所見から,胆囊捻転症と診断し,緊急腹腔鏡下胆囊摘出術を施行した.術中所見では胆囊は腹壁から肝表面へ繋がる索状物で絞扼されており…
