62歳，女性．健診の心電図異常と労作時息切れを主訴に外来受診した．冠動脈CT（computed tomography）で右冠動脈は著明に拡張し，左冠動脈は肺動脈から起始していた．負荷心筋シンチグラフィでは前壁心尖部領域に虚血所見を認め，冠動脈造影で左冠動脈領域は右冠動脈末梢より逆行性に灌流され，肺動脈への流出が確認された．虚血症状を有する左冠動脈肺動脈起始症（Bland-White-Garland症候群（BWG症候群））に対して左冠動脈再建術を行った．
