症例は61歳の男性で，左鼠径部腫脹を主訴に受診した．同部は小児頭大に腫脹し，CTで巨大な鼠径ヘルニアおよびヘルニア囊内と腹腔内の小腸に造影不良を認めたため緊急手術を施行した．鼠径法，下腹部正中切開を行い鼠径部ヘルニア分類（新JHS分類）L3型ヘルニアおよび小腸に250 cmに渡り色調・蠕動不良を認めたが，改善傾向であったため温存し，狭窄・壊死などによる再手術も考慮しヘルニア門は解放のまま待機的手術の方針とした．術後は下血，炎症反応の高値がみられたが保存的に改善し，初回手術後…
