包括的高度慢性下肢虚血（CLTI: chronic limb-threatening ischemia）の治療には全人的な管理が必要で，多職種による集学的治療が鍵となる。看護師の診療補助業務が拡大適用され特定行為として一部の医行為実施が認可された。担当医師との情報共有がより重要となる中，在宅ケアへシームレスに移行する方策には看護の視点からメディカルスタッフを含めた他職種者とともに検討することが必要である。治療とケアの融合を課題とした実践内容を報告する。
