74歳，女性．発熱，全身倦怠感，腎機能障害のため，紹介された．腎生検にて肉芽腫性間質性腎炎と診断．眼病変や血液検査，画像検査の結果からサルコイドーシスと診断し，サルコイドーシスによる腎機能障害と考えられた．ステロイドを開始し，腎機能は改善傾向である．サルコイドーシスにおいて，正常Ca値でも肉芽腫性間質性腎炎による腎機能障害を呈する例もあり，可能な限り腎生検を施行し，診断を確定する必要がある．

14歳，男児．剣道の練習後に肉眼的血尿と尿蛋白を認め，受診した．部活動後に限定された肉眼的血尿というエピソードや溶血所見より，剣道にて誘発された行軍ヘモグロビン尿症候群の診断に至った．腎炎合併を否定するため施行された腎生検では近位尿細管への鉄沈着を認めた．若年者の運動後に検尿異常を認めた場合，運動歴の聴取が診断には必須である．予防により血管内溶血を軽減させることが可能であるため，予防策の提示も重要である．
