症例は腹部手術歴のない44歳の女性.持続する腹痛と嘔吐を主訴に当院を受診した.腹部CTで左下腹部にclosed loopを認めたが,血流障害の所見は認めなかった.内ヘルニアによる腸閉塞と診断し,イレウス管で保存的加療を施行した.その後も腸閉塞の所見は改善せず,イレウス管留置後3日目に手術を施行した.腹腔鏡下に観察すると,S状結腸間膜の背側に膨隆とS状結腸間膜窩に小腸の嵌入を認めた.S状結腸間膜窩ヘルニアと診断し,直径約2cmのヘルニア門を開放し嵌頓解除を行った.腸管切除は施…
